Sign-in sheet template
A simple attendance sheet that works on paper or on a tablet at the door. Twenty-two rows per page for name, organization, contact details and times in and out.
Fillable PDF · US Letter · 97 KB. Made by FormFlow; free to use, change and share.
Paper or tablet
Print it for a clipboard at the door, or open the PDF on a tablet in FormFlow's Fill PDF form tool and let people type their own row. Fillable rows mean the finished sheet can be read by a computer later, which handwriting cannot.
Collect only what you need
A sign-in sheet is visible to everyone who signs after you. Ask for an email or phone number only if you will actually follow up, and tell people why. For visitor logs in offices or clinics, consider a separate private form for anything sensitive.
After the event
Use Extract form data to turn one or many filled sheets into a CSV spreadsheet for your mailing list or attendance records. Then flatten and archive the PDF, or delete it if you no longer need it.
Tips
- Write the event name and date before printing so pages never get mixed up.
- Use one sheet per day for multi-day events.
- For continuing-education credits, add a signature column in Create fillable PDF.
